I am running a short survey on user choice - I wish to keep it as vague as that for now so as not to colour the results.

Some people have mentioned it looks slightly like a phishing attack. Just to set minds at rest here is a short privacy brief:
- Email addresses are not linked to IP address, and are totally optional
- Passwords ARE stored in plain text. Your encouraged to make a new one for this purpose
- IP addesses are stored only to prevent you taking the survey more than once - they will be deleted when the survey is closed
